New Nationwide Policy on Facet Joint Interventions!

Interventional pain management techniques in general, facet joint interventions, and epidural interventions in particular, have been in the news for the past year. There have been 2 reports from the Office of Inspector General (OIG) covering the frequency of therapeutic facet joint injections and another report from Noridian with only 49% of the claims meeting appropriateness criteria.

Based on the Cures Act to provide uniformity and transparency, new local coverage determinations (LCDs) have been established. Consequently, a multijurisdictional expert committee had hearings on potential coverages on the future for facet joint interventions in May 2020 and for epidural interventions in February 2021.

ASIPP UPDATES

ASIPP is pleased to announce the publication of the Comprehensive Evidence-Based Guidelines for Epidural Interventions in the Management of Chronic Spinal Pain. In preparation of the guidelines, an extensive literature review was performed. The 210-page guidelines contains 33 figures, 48 tables, 1,345 references and has 60 authors.

In addition to the review of multiple manuscripts in reference to utilization, expenditures, anatomical and pathophysiological considerations, pharmacological and harmful effects of drugs and procedures, for evidence synthesis, we have included 47 systematic reviews and 43 RCTs covering all epidural.

The American Society of Interventional Pain Physicians has received numerous requests on potential guidance regarding the use of steroid injections during COVID-19 and specifically guidance on the use of steroids in injections, before and after being vaccinated. We are currently working on finalizing this document and would like to share the draft version with you. We will update you soon with the final version.

Placebo effect is fascinating, yet totally misunderstood and biased. A placebo is a substance or treatment, which is designed to have no therapeutic value. Essentially, it means an inert substance is injected into an inert structure through injection other than by mouth or sham surgery. The use of placebos in clinical medicine raises ethical concerns, especially if they are disguised as an active treatment, as this introduces dishonesty into the doctor-patient relationship and bypasses informed consent.

Amniotic Tissue Intraarticular Injections

Many ASIPP members have recently been approached by sales representatives claiming that amniotic or umbilical cord tissue products are now covered by Medicare and many private insurers for intraarticular injections and interventional pain management procedures.

Based on collective evidence, unfortunately, these claims appear to be INACCURATE, subjecting patients and providers to high risk.

Amnion is covered for limited cutaneous wound healing applications and for a handful of neurosurgical and ophthalmic surgical uses, it has a product reimbursement code (also known as a Q code). If a physician submits the right combination of ICD-10 and CPT codes, Medicare may pay these claims.

Based on review of Medicare local coverage determination (LCD) documents, biologics for intraarticular injections and interventional pain management procedures are Experimental and Investigational. Hence, there is NO coverage for these products for intraarticular injections and interventional pain management procedures. 

It’s the recommendation of ASIPP that you avoid billing Medicare and private insurance for these products when used to treat orthopedic and pain management conditions. Our concern is that by billing these products to a federal program like Medicare, not only could there be significant Medicare clawbacks, but knowingly billing Medicare for products that you know are not covered can result in serious criminal penalties. Even, initial losses could be extensive.

For additional information on coverage guidelines from many private insurers that state that these products are NOT covered, experimental and investigational. Please check the websites of commercial carriers.

Pain Physician, the official journal of ASIPP, has launched a special issue covering COVID-19 issues. The publications range from understanding the pathophysiological mechanisms, epidemiology, implications on interventional pain practices, safety and effectiveness of various modalities of drug therapy, telemedicine, along with the effect on interventional pain management practices, and, finally, the technological impact of COVID-19 on the future of education and health care delivery and multiple guidelines.

Evidence-Based Risk Mitigation Strategies and Stratification during COVID-19 for Return to Interventional Pain Practices [ https://pubmed.ncbi.nlm.nih.gov/32942784/ ], and Triaging Interventional Pain Procedures during COVID-19 or Related Elective Surgery Restrictions [ https://pubmed.ncbi.nlm.nih.gov/32942785/ ]. 

Both manuscripts describe various means to continuation of the services at present, and also in future if there is a resurgence or in a worst-case scenario lock-down, and training and education during COVID epidemic. Other articles include Burnout among Pain Physicians, Implications on Interventional Pain Practice, and manuscripts on antibody testing, drug therapy, and stem cell therapy.
